Exclusive Provider minimal requirements for FULL CUSTOM Mouthguards™:

1. Upper metal Rim-Lock Trays® by Caulk; U11 (qty. 2), U12 (qty. 4), U13 (qty. 4), U14 (qty. 2). Pearson Dental has the best price.  Tallest vertical anterior wall compared to any impression trays. Excellent for mouthguard, partial and full denture, and whitening tray impressions. Roll borders, vestibule, and frenum attachment can easily be achieved without over filling tray.
2. Rope wax, red.
3. Jeltrate® alginate, regular set. Not fast set, don’t learn the hard way.
4. Jeltrate® water / powder measurement guide. This does not come with your alginate.
5. Tupperware; use as a humidor if you can’t pour impression immediately.
   All alginate impressions are to be poured within 30 minutes.
6. Rubber Model Base Formers (LRD), TWO SETS, Pearson Dental item # L15-0150. Use for Mandibular Impressions ONLY.  This is the best value! Contains 3 uppers and 2 lowers. You’ll most likely be only using just the large upper and large size lower, the rest are too small, and that’s why you need two sets. They are not sold individually- but still has the best price, compared to the other brands sold separately.
7. Hardrock™ (Whip Mix), available in aqua or green. You can use the ‘cheaper’ yellow stone for base.
8. Wax spatula #31 (optional). Use to appropriately place stone in alginate. Please round pointed end with greenstone bur and polish with brown rubber point.
9. Opti-Bite (Pac-Dent) bite registration material, fast set, available through Pearson Dental. Best value! It’s the cheapest and easy for lab tech’s to work with; not brittle.

If you do not meet the minimal requirements, can not knowledgeably take FULL maxillary impressions and can not properly provide adequate molds, you will not be listed as an Exclusive Full Custom Mouthguard™ provider. NO EXCEPTIONS.
